Should the amplitude of the waveform be adjustable by both the input and output volume sliders? Mine is.

The Windows control panel sliders are schizophrenic. They change depending on what you're doing (it's what makes Windows so darn much fun).
What are you doing and on which Operating System?

Don't bother. Only CDex is a full ripper. Anything you do with Audacity is likely to cause damage or odd results.
