The most likely is Windows Enhanced Services, the conferencing processor.

Audacity 2.0 is several years old. It is not the cause of your problem but I suggest you update to Audacity 2.1.0 from http://audacityteam.org/download/windows.

Resolved the problem by changing my default playback format from 24 bit 48000 Hz to 24 bit 192000 Hz.

On the other hand, 192000 Hz is more demanding for the computer, and 44100 Hz and 48000 Hz “should” also play fine. If the bad playback was in another specific app, it may itself have been set to 192000 Hz, creating a mismatch problem.
If problems come back again and enhancements or sound effects are not the cause, you may want to check if you have correct and up-to-date sound device drivers.
