Sample Printer is shipped with Audacity 2.0.1 and later.
See "Sample Data Export" in the Audacity manual for instructions for the current version.

You need to create separate "tracks" on the CD.

It's unlikely that you did anyting to damage your soundcard... There might be something helpful on this page . I'd expect the UCA 202 to work well with your set-up though you'll need to be careful not to push the output level from your mixer too high.
