What input are you trying to record from? Line In, Mic In, Stereo Mix, etc.? If you're trying to record a Stereo Mix signal and this wiki page doesn't help you then you will probably need new or drivers that supply that signal. What you're describing is exactly what happens when you import a WMA or MP4 (aka m4a) file. These are compressed formats that Audacity cannot currently read.

You'll have to go through Windows audio setup software to change your input to Line In (or Mic In if that's what you're using).

Sound cards can die. I've seen it happen. It's rare though.

Does the Echo Indigo use ASIO drivers? Audacity cannot use those for legal reasons (somebody kick Steinburg for me). I noticed that you default to using Outs 3+4. If Audacity isn't recording in stereo, and it's set to record 2 channels, then it isn't receiving a stereo signal from the source.
