Delete the per-user configuration file:
Windows: Documents and Settings<user name>Application DataAudacityaudacity.cfg
OS X: ~/Library/Application Support/audacity.cfg

Sound Drivers - it usually is on Windows, especially if invasive things have been done to the multimedia stack. I can't offer much help, except to ask you to try what happens with audacity 1.3.3, which has updated audio I/O code, and see if that works.

Two options:
1) Move each recording along it's track to start as the previous one ends, using the time shift tool. They will all get merged vertically anyway when you export the project
2) Use 1.3.3, where you can do drag and drop between tracks to move clips around.

Cable fallen out of computer? Different input on your sound card selected to the one you are actually using? Mic input muted when using some other audio program?

Use the scroll bar to move until the ruler at the top has 15 mins on the screen (zooming in or out may help), then click on the waveform to position your cursor there. In 1.2.x, yes. In 1.3.3 you can drag clips between tracks, but that depends on multiple clip per track support, which is only in 1.3.x

You need to patch each user's registry, because audacity reads a registry key to find out where the lame DLL is stored (as you are on windows). That key is in the per-user part of the registry along with all the other audacity settings, to avoid needing administrative rights to install audacity. Giv...
