Peaks don't correspond that well with perceived loudness, and with vinyl transfers you can have a "click" in one channel that represents the loudest peak. Noise reduction isn't perfect... You can experiment with the settings and get "more agressive". I assume you gave Audiacity a sample fo noise-only for the noise profile? If you go too far, "The cure can be worse than the disease" , but you may be able to use a more agressive setting only on the fade...

Since at least two applications have failed to recognize the blank CD, I'm going to guess that your drive is bad. :( I assume you've tried more than one blank CD-R, and more than one CD-RW? With a problem like this... with most computer problems... you don't know what the problem is 'tll it's solved...

I don't know about VST... they are scattered all over the Net (both free and commercial). But, if you didn't already download the LADSPA plug-ins, there is a "pak" of 90+ of them on this page.

If you open a WAV file and you don't do anything (no dithering or anything), other than save it under another name, the audio data remains unchanged. It gets converted to 32-bit floating-point for internal use in Audacity, but these 32-bit numbers can be converted-back to 16-bits without changes and...

As I'm sure you know, MP3 is a lossy format... That means the encoded/decoded data is different from the original uncompressed data. The peak level is one of the things that can change... If you go to Effect -> Amplify and set the New Peak Amplitude to 0dB, that will normalize (maximize) the volume. It depends on the format... Compressed formats are "rated" in kbps (kilo bits per second). Uncompressed files (i.e. WAV) depend on the number of samples (i.e. 44.1kHz - 44,100 samples per second) times the bit depth times the number of channels. There are 8 bits in a byte, so an uncompressed 44.1khz...
