Fixed it!

Had to switch from MME to Windows DirectSound in Edit->Preferences->Interface, and then select SoundMAX Digital Audio for Playback and Recording.

Options for Interface are MME and Windows Direct Sound. Playback options are Microsoft Sound Mapper, Modem #0 Line Input, and SoundMAX digital audio. I've selected SoundMAX as default.
