Make sure that USB PnP recording device is the Tape Express. If you are not sure, open Sound in the Mac's System Preferences, then disconnect and reconnect Tape Express and observe the name of the device that disappears and reappears. Yes. Most applications would use libvorbis to encode OGG files, as Audacity does.

I'm nearly positive there isn't a simple, common sound format with chapters. AAC explicitly supports chapters. The 1.7.1 Queen Mary plugins crash Audacity, as it says in the 2.1.2 Release Notes. Follow the instructions in that link to install the older 1.7 plugins instead.
