Make sure that USB PnP recording device is the Tape Express. I'm nearly positive there isn't a simple, common sound format with chapters. AAC explicitly supports chapters. The 1.7.1 Queen Mary plugins crash Audacity, as it says in the 2.1.2 Release Notes. Follow the instructions in that link to install the older 1.7 plugins instead.
