Make sure the built-in audio output in Apple Audio MIDI Set Up is set to 44100 Hz 16-bit. See here.

Check the speaker or headphones jack for faulty connection.

If this is an Intel Mac or OS X 10.5 +, the latest Audacity Beta (currently v.1.3.12) is generally more recommendable.
