Are there small pieces of the recording missing, or is the speed incorrect? Perhaps you could attach an audio sample so we could hear the problem.
What is the make and model number of the USB device you are using?

Please describe your setup in more detail. What playback and recording devices have you chosen in Audacity? Why would you use a USB mic when you have a USB interface to which you could connect a powered studio mic? Have you installed Skype? That can interfere with levels of audio devices. There are ...

Yes it's a bug as noted in the 2.1.2 Release Notes (third bullet down from where the link lands). The content is correct for your chosen format. This will be fixed on Mac in the upcoming 2.1.3 release of Audacity. However, using back or forwards slashes in file names is a bad idea if these files are intended for use on the internet or for people on other systems than Mac.

Thanks for letting us know. The upcoming Audacity 2.1.3 will be codesigned with an Apple Developer ID and with some other precautions this problem should not happen when launching Audacity. It might still happen when closing project windows.

Changed my audio to buffer speed from 100miliseconds (up to 150, then 200ms On Mac, that is the wrong direction. You may need to reduce it, perhaps to zero.
