I think this fixes all of the bugs. This version dispenses with the band-pass / pand-pass options so as to simplify the interface. Yes, though I don't expect many people will open multiple pan / gain control dialogs at the same time.
I'd rate it as P4. (I would have said P5, but as it's a regression I'd bump it up to P4).
