I found a computer at work that had Realtek HD on board, so I enabled it so that I could have a look. To record a stereo track you need to set stereo recording in preferences:
"Edit > Preferences > Audio I/O" "Channels = 2 (stereo)"

I don't use iTunes myself, but from what I've heard, I thought that you could convert from MP4 to MP3 in iTunes.

WC's method is simple and should work well, but as he suggested it is possible to "Rip" the audio from a DVD, but it is a little bit complicated to do. There are tutorials, and software available from the "Doom9" for doing this.

I didn't realise that Hydrogen had been ported to Windows, I've only seen it on Linux. But yes, on Windows you need to set the WINDOWS MIXER to record from "Stereo Mix" (may also be called "Mix", or "What U Hear", or something along those lines depending on your hardwar...
