I know what you mean - for us that are used to analogue and 16 bit, we like to maximise our signal close to the red line, so every bit of extra headroom is great. This is usually a symptom of either: A computer crash - Audacity closed before all the data was saved. or Some of the data files have been moves, changed, or deleted. Windows Media Player is known to have issues with some MP3 ID tags. For playing audio files, Foobar2000 is much better (if less pretty) than WMP, and can also be used for encoding/decoding various audio formats.

Clipping can occur anywhere in the input chain, from the microphone, input of the pre-amp, output of the pre-amp, ... You need to find out where in the signal chain the clipping is occurring. If you have labels that mark regions (rather than single point labels) then you can select the region quickly by clicking on the label name. This makes the "brute force" method a lot quicker and easier.
