Did you previously have Audacity 1.3.x installed and associated with MP3 files?

There's no metadata in that file that I can see.

You only need wxWidgets installed for building Audacity from the source code.

Ctrl+M to create a label while recording.

I'm not sure what is happening there, but Audacity does not support album art. MP3s created by Audacity do not have album art.
Perhaps the low resolution image is not actually embedded in the file, but just being displayed by your media player somehow?

Try exporting as FLAC.

Check that playback is "Stopped" and not just "Paused".
