I've no idea why you should be having so much trouble (unless perhaps the CD is scratched or something). Programs like CDEX and EAC should have no problem ripping a CD accurately and VLC, foobar2000 and such like should have no problem playing the ripped WAV files, but since you are having this prob...

What do you have set as the output (playback) device in the Device Toolbar?
If you import a "known to be good" audio file into Audacity and play it, can you hear it?

A bit of explanation about how the Noise Removal effect works. When dealing with a good recording (the background noise is pretty quiet), the noise is only really noticeable when the sound level is very low. I'd probably go a little less aggressive on the noise removal - put up with a bit of hiss and avoid the swooshing sound.
