What operating system are you using?
Which version of Audacity? (look in "Help > About Audacity")

Have a look at "Replay Gain" (https://en.wikipedia.org/wiki/ReplayGain and http://wiki.hydrogenaud.io/index.php?title=ReplayGain).
iTunes uses a similar (but proprietary) system called "Sound Check".

I don't know what you mean. The "latest version" of the "Level Speech" effect does not have a "threshold" setting.
