Can't really blame MediaInfo: the file header says that the sample rate is #00007D00, which is 32000 Hz, though from listening to the file, that is clearly not the case.

If you are using Audacity 2.2.0:
open "Help menu > Diagnostics > Audio device info".
Save the info when it appears.
Attach the saved info file to your reply.

The record button will be greyed out while Audacity is playing. If you want to record, stop playback first, then click the record button and Audacity will begin recording. Reading that again, what do the numbers say on the vertical ruler at the left end of the audio track? Have you just zoomed out vertically? http://manual.audacityteam.org/man/zoom ... al_zooming

I presume that you are recording to a stereo track? (in the device toolbar, the "number of channels" set to "2 (stereo)".

Check in the Windows Sound Control Panel that the recording device is also set to record 2 channel stereo.
