I suppose the first thing we should address is that if you're Mac/Intel/Leopard (a new Mac) you have the wrong Audacity. The upgrade from Tiger to Leopard (10.4 to 10.5) pretty much killed off Audacity 1.2.5 for us. If you are running Audacity 1.2.6, I wouldn't expect anything to work right, that's ...

The theory is pretty simple. Convert L and R to L+R and L-R and then back. What you do with the L-R signal in the middle -- plus or minus amplification -- affects separation. Sometimes Preview is part of auxiliary tools that aren't supported. If you select a small part of the show and actually apply the tool and then Edit > UNDO, does that work?

You can also get that when you're running out of drive space. How large are the drives and how full are they?
This isn't mentioned in the wiki page. Do you ever error check and defragment the drive? Audio and video production programs put a serious stress on system resources and speed.
