The first place I'd look is in the Apple Preferences. Apple > System Preferences > Hardware > Sound > Output. Make sure the output sound is routed to the Built-In Speakers. That should give you your iTunes, QuickTime, etc, back. You can cause significant drama if you like to use Skype, SoundFlower, ...
