As recording a telephone call is notoriously difficult, how are you doing it? If it involves SoundFlower or other channel management software, did you leave Audacity Playthrough running by accident?
Audacity > Preferences > Recording > Playthrough [_] (Deselect).

The iPhone and the computer Headphone are both trying to push audio to each other -- clearly not going to work. And there's no question why you want to do this. We recommend keeping the production sound elements separate if you can.

Have you ever done Go > Desktop > Utilities > Disk Utilities > <your system drive> > Verify Disk and Repair Permissions? Actually you can do that to all your connected drives although they will only allow Verify. If you have puzzling permissions issues, that maybe worth a shot. Koz

Another oblique idea. Is your device muli-channel over 2? Are you expecting 6 or 8? The system settings should match each other whatever they are.

I'm still fascinated by you getting the Mac System Preferences to work and yet Audacity fails, even though it recognizes the device. That's extraordinary. You are at 44100, 16-bit, stereo right?
