When I do the Amplify test after Normalizing, it shows 3.0 But if you do that to an older original raw recording , I bet it shows around 3.0 or very close to it. I also bet if you do that to your mystery raw recording, it's much louder than that, say closer to 1 or even 0.5. That would account for ...

P pause and Append Record (Shift-R) are the two known ways to continue at the end of an existing track. Is turning the volume down the only way to prevent this? Yup. As above, when you're under the protective umbrella of Audacity, that's not serious damage, but the instant you leave, the damage becomes apparent and it's permanent. It may not take much volume reduction to solve it. Leave Show Clipping...
