The requirements for AudioBooks, at least from ACX, are stiff. I would expect whatever you plug into the side of your computer to fail one of the three specifications. The mono ACX-Check you posted was for the whole chapter?
Do you have Chapter 1 posted somewhere we can see the WAV version? I think that's the only way I can give you whole chapter correction.

You should be working in Mono rather than struggling with two-channel stereo. Everything is easier or more efficient. I got it to pass with relatively simple corrections. How are you listening? You're supposed to set the playback volume for comfortable listening and then roll into the quiet segment without touching anything. Close Audacity.
Go > Go To Folder > ~/Library. Open Application Support > audacity. Drag audacity.cfg to the trash.
This is not a burn it off solution. If you have a bad plugin or add-on, it will still be there when you reopen Audacity.
