Not saying they are the cause every time but, phone chargers often emit loads of hash via radiation and back onto the mains wiring. Yes, you mic each person with a lav mic and feed those to the camera. I'm assuming the camera has a left and right input. You then record each person per track and mix down in post. Does it have a HDMI "live" output and does it carry the audio? If so, get a HDMI de-embedder/audio extractor and feed that to a small headphone amp. As Steve mentioned, I would start with better quality cables. What I would also do is, switch off everything else that is connected to the same extension/plug or close proximity. Over and above what Steve wrote about 50Hz (mains), there is also a strong 2nd harmonic at 100Hz and other noises going up to around 7KHz.
